Information Collection, Use, and Sharing survey data through "Allergy and Asthma Wellness Centers" owns the information collected on this website. We only collect information that you voluntarily give us via this website or email or other direct contact from you. We will not sell or rent this information to anyone. Only non-identifiable information is gathered through this website. This information will be used for research purposes only. We do not collect contact information via this website.

Security
We take precautions to protect your information. When you submit sensitive information via the website, your information is protected both online and offline. Information collected at this website is encrypted and transmitted to us in a secure way. You can verify this by looking for a lock icon in the address bar and looking for "https" at the beginning of the address of the Web page. While we use encryption to protect sensitive information transmitted online, we also protect your information offline. Only personnel who need the information to perform a specific job (for example, school nurse facilitating surveys, researchers using data for reports) are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are kept in a secure environment separate from this website.

In addition to the survey questions, you will be asked for some personal information. While this information is personal in nature the questions are not considered personally identifiable (PII). The following represents the information you will be asked or will be derived:

• Participating School and/or Organizations ( derived from web address/URL if provided, e.g. school.aaws-pas.org)
• Gender
• Age (not birthdate)
• Grade in school
• Zipcode
• Ancestry
• Type of Insurance
• Participant code (see Registration section below)

Minors
If you are a minor taking this survey, or if you are a parent/guardian taking the survey on behalf of a minor, you must follow local, state, and federal law: http://www.nasbe.org/healthy_schools/hs/bytopics.php?topicid=6130.

Registration Registration is not mandatory. If you’ve been given a participant code you may enter it at the end of the survey if you wish. This participant code will be used by Allergy and Asthma Wellness Centers to: 1) track surveys over time, 2) link surveys to participant information that is stored securely separate from this website. To correct any inaccuracies in information that you entered into the survey you may retake the survey and enter the same participant code.
